TB Skin Testing:

• TB Skin Testing is available on the following days: Monday, Tuesday, Wednesday and Friday (No TB Skin tests on Thursdays) 

Note: Test must be read 48-72 hours after given-please plan accordingly

• Cost: $15

Head Check (Lice):

• Free head checks for lice are available during our walk in clinic hours. 

Immigration Vaccination Forms (I-693):

• The immunization clinic can complete Part 1 and the vaccination record (page 5) of your I-693 form. 
• Cost: $39 per person

  • All applicants must be present with a photo ID
  • I-693 Form 

**Note: It may take up to a week to complete your form and there may be additional charges for any required immunizations that you may need.**

Monday, Tuesday and Friday: 9:30a.m.-3:30p.m.
Wednesday: 9:30a.m.-4:00p.m.
