Willow Health Center is located at the 1115 Penn Building. Willow Health Center provides the following services to adolescents ages 5-25:
Primary Health Care
Licensed doctors and providers at Child and Adolescent Health Centers offer primary health care services, including:
- Annual/Sports/School/Work physicals and Well Child exams
- Treatment for minor illness and injuries
- Vision/Hearing screenings and follow-up
- Chronic disease management (Asthma, diabetes, obesity, etc.)
- Immunizations
- Basic lab services and tests
- Counseling and behavioral health services
- Health education
- Referral to community services
- Referrals for specialty services
Primary Health Care
Child and Adolescent Health Centers offer the following confidential services to mature minors:

Pregnancy testing

Sexually transmitted infection (STI) and HIV screenings, treatment and counseling

Substance abuse education, counseling and referrals

Crisis intervention and emergency care

Mental health assessment, counseling and referrals

Physical/sexual abuse counseling and referrals
Behavioral Health
Licensed Social Workers offer screening and assessment for a variety of behavioral health issues facing adolescents today. Comprehensive screening and assessments are available to individual patients at each of our Health Centers, and to members of the community.
Health Education and Outreach
Child and Adolescent Health Centers host evidence-based workshops for students and community partners around STI’s/contraception, nutrition, bullying, dating violence, depression & suicide, and homeless prevention. All workshops are at no-cost and available to the community.
Family Planning
Family Planning (birth control and contraceptive) services are available ONLY at Willow Health Center. Child and Adolescent Health Centers also provide services for the whole family, including free Medicaid Enrollment Assistance and appointment scheduling for medical or dental services for all ages through Ingham Community Health Centers.

Welcome to Ingham Community Health Centers! Below are many of the forms that you will encounter during your time as a patient with us. These forms allow us to treat you, receive or send your medical records, help you pay for care, and more. If you are interested in becoming a new patient, or need to update your information, please fill out the first form in the list.
Global Consent: This form gives your consent for treatment at our Health Centers, billing for services, and sharing of healthcare information in the event that you decide to receive behavioral healthcare with us.
Registration Form: This form captures all of the information that we need from you to establish care, or update your existing information with us.
Family Income Form: This form allows us to determine if you are eligible for a discount on the healthcare you receive from us.
Minor Consent: This form allows us to provide care for your child at one of our school-based or school-linked clinics.
