Community Health Improvement Plan (CHIP)

Ingham County Health Department  (ICHD) promotes community health by monitoring health status, identifying major health problems and engaging  stakeholders in partnership to develop strategies to achieve the highest possible level of health to local residents. The Community Health Assessment unit of ICHD convened a core coordinating staff group to follow up on the Healthy! Capital Counties Assessment. The group then engaged stakeholders in a steering committee to develop a Community Health Improvement Plan (CHIP). The CHIP includes  common goals for five areas of needs,  evidence based objectives and strategies, and specific tactics on how to implement the strategies by  committed organizations and leaders in the community.

  • Improve Behavioral and Mental Health 
  • Reduce Chronic Disease
  • Reduce Obesity
  • Promote Financial Stability and Economic Mobility
