Mandated Reporting and Health Department Services for STDs and HIV/AIDS

- The completed two-sided form can be faxed to (517) 887-4663 or reported by phone to (517) 887-4424. You can also request access to the Michigan Disease Surveillance System (MDSS) as a reporting tool by calling (517) 887-4308.
- Upon request, Health Department Investigators will assist in locating clients who have not been treated within one week of a positive report.
- If a client cannot afford treatment, they may be referred to the Health Department.
