The Public Health Nursing Division provides community nursing, nutrition education and social work services in homes, schools and other community settings. These services are directed at preventing health problems in high risk populations. Assessment, coordination of care, health education, and referral to appropriate community resources are key components of the services. Referrals are accepted from medical provider’s offices, hospitals, community agencies, and individuals may refer themselves.
Specific key programs include:
Maternal Infant Health Program
House Calls - Tobacco treatment for pregnant and parenting women
Infant Mortality reduction programs focusing on safe sleep, interconception care, prenatal care and decreasing health disparities.
Early On service co ordination
Lead poisoning education and follow up
School Health Services
Nutrition counseling for all ages
Social Work support services
