Plan Review

                                                  Plan Submission Instuctions Checklist

Whenever you are proposing to build or remodel a food establishment in Ingham County, there is an 8 step plan review
process to complete.  The follwing 8 step process must be completed and compiled into a single package and submitted
to the Ingham County Health Department, Bureau of Environmental Health.  If all items are not submitted, the plan
review process may get delayed as additional materials are requested.

PLANS MUST BE APPROVED BY INGHAM COUNTY HEALTH DEPARTMENT PRIOR TO BEGINNING CONSTRUCTION.  

8-Step Plan Review Process

1.    Plan review application/transmittal letter and any neccessary plan review fees.  These documents are included in the         
       Plan Review Packet.

2.    Completed plan review worksheet
       Worksheet and guidance information are available in the Plan Review Packet and also on the MDA website at

3.    Menu
       Submit a proposed menu
       If serving undercooked eggs, chicken, meats, seafood, etc., provide information on Consumer Advisory

4.    Standard Operating Procedures (SOP's)
       SOP's appropriate to your operation shall be submitted prior to opening.  See the Standard Operating Procedures 
       Manual guidance document available in the Plan Review Packet for further details.

5.    Certified Manager Documentation
       Prior to opening, submit documentation verifying that an individual certified in food safety under the American National
       Standards Institute (ANSI) accredited certification program (Michigan Food Law of 2000, as amended, Section 
       289.2129), will be employed at minimum 30 hours per week in you establishment.

6.    One complete set of plans.
       Provide scaled plans (1/4" per foot is norman, easy to read scale).  Include the following:
             •  Floor plan, with equipment identification
             •  Mechanical plan, including:
                        ∠  Make-up air systems
                        ∠  Air balance schedules
                        ∠  Cooking ventilation systems: including duct and exhaust fans
             •  Plumbing plan, including:
                        ∠  Sanitary waste
                        ∠  Potable water piping
             •  Lighting plan (including shielding)
             •  Site plan, including:
                        ∠  Outside garbage storage area and containers, as well as exterior storage area
                        ∠  On-site water well and sewage data (if applicable)

7.     Specifications
        Include manufacturer's specifications for each piece of equipment.  Minimum information for each piece of equipment
        includes the following (note: the manufacturer's specification or "cut" sheet typically provides most of this 
        information):
             •  Type of equipment
             •  Manufacturer's name
             •  Model number
             •  Dimensions of the equipment
             •  Performance capacity
             •  Indicate how equipment will be installed (i.e. on leg or wheels, fixed or flexible utility connections)
             •  Indicate which items are used equipment and what equipment is NSF approved or equivalent.

8.    If you are proposing a brand new facility, include the Food Service License Application.  This document is included in
        the Plan Review Packet
