Mandated Reporting and Health Department Services for STDs and HIV/AIDS
Identifying, treating, and reporting of STDs and HIV/AIDS is an important health concern. Michigan Communicable Disease Rules requires that STDs be reported within 24 hours and HIV/AIDS within 7 days.

Michigan mandates that the following STDs and their treatment be reported utilizing form “MDCH STD DCH-0821A”:

Chlamydia Trachomatis

Gonorrhea
-
Syphilis
-
Chancroid

The completed two-sided form can be faxed to (517) 887-4663 or reported by phone to (517) 887-4424. You can also request access to the
Michigan Disease Surveillance System (MDSS) as a reporting tool by calling (517) 887-4308.

Upon request, Health Department Investigators will assist in locating clients who have not been treated within one week of a positive report.

If a client cannot afford treatment, they may be referred to the Health Department.
